The 21-year-old's agent claims an agreement has been reached with all parties interested in the Belgium international and the final decision now rests with the player himself.
The Lille playmaker is one of Europe's hottest properties and he has been closely linked with a summer transfer to Barclays Premier League rivals Manchester City, Manchester United and Chelsea.
Lille managing director Frederic Paquet said earlier this week that he believed there were four clubs leading the chase for Hazard but said he did not know where the player would be moving to.
Hazard's agent John Bico says terms have already been agreed with all suitors, saying in the Daily Telegraph: "We are no longer in the negotiations stage. We have reached agreement with all parties. There will be no higher bid.
"Eden has already said he wants it to be fixed before June 2. He will decide after the games against Montenegro and England, then go on holiday with peace of mind."
Belgium play Montenegro in Brussels on Friday and then take on England at Wembley on June 2.
The Belgian wonderkid signed off in Ligue 1 with a hat-trick as Lille beat Nancy 4-1 before it was expected he would announce which club he would be leaving for.
Following the game, the 21-year-old refused to give anything away but admitted Chelsea's Champions League victory had made him 'hesitate' over his final decision.
"As long as I've not put pen to paper, I won't say where I'm going," the Belgium international told Canal+.
"Chelsea makes you hesitate. They've won the Champions League and they'll be in it next season.
"It's important for me to play in that competition. Chelsea are a great team with great players. Of course it's a possibility.
"For now nothing's been decided but it will be soon. Now I want to join up with the national team and then go on holiday. It'll do me a lot of good.
"It is possible that Chelsea's arrival at the negotiation table will delay things, as nothing is done," he added.
"My agent has met with Manchester and with Chelsea. They both give good offers and are both great clubs. I don't know. It's not been finalised yet."
